First Visit

Please assist us by bringing the following information at the time of your consultation:

  • Your insurance card(s) and photo identification
  • A list of medications you are presently taking including the dosage (ex. 10 mg), and how often you take it (ex. 1 pill twice daily). Also, a list of the over-the-counter supplements (vitamins, minerals and herbs) you are taking including the name and dosage.
  • A list of all medication allergies and the associated reactions
  • A list of all past surgeries including the year it was performed
  • If the patient is a minor, bring vaccinations record
  • You will be responsible for your co-pay at arrival
  • Download, read, sign and bring the New Patient Forms (see link below)

Office Visits

All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

We strive to see all patients in a timely manner. However, medical issues arise from time to time. Delays may occur and appointments may need to be rescheduled. We understand your time is valuable and we appreciate your patience and understanding.

Office Policies

All co-pays, deductibles and other payments are expected on the day of service unless prior arrangements have been made with the billing manager. Co-pays are determined based on the rate as set by your insurance provider. For your convenience, we accept VISA, MasterCard and Discover.

Our doctors are not in the office every day. Therefore, please be mindful of your prescription refill requirements. Approval of your refill may take up to three business days. Additionally, medication refills will only be addressed during regular office hours (Monday-Friday).

As a courtesy to our patients, we don’t charge a fee to complete FMLA notices, disability paperwork or absence forms; however, we do require five (5) business days to complete any forms requested of our staff members.
