Preventive care, end to end.

Preventive care is medical care with a focus on disease prevention and general well-being through regular check-ups and health screening. An annual exam is an important part of your medical care. We offer a range of preventive care appointments below.

i.

Adult Medical Exam.

Annual physical

At your medical exam, we will review your medical and family history. We'll take and record your vitals. At this exam, you will discuss your health concerns and risk factors with your doctor. An examination will be performed. Your doctor may run a series of tests and blood work, as needed. Your doctor will discuss your health and any treatment options with you.

Please bring a list of all your current medications with you to your appointment.

Diabetes Assessment & Risk Screening.

Screening

Diabetes is a group of diseases in which the body's inability to produce enough insulin can result in too much glucose. Depending on your medical and family history, you may be at risk for developing diabetes. Your doctor will assess and monitor your health for risk factors. When necessary, your doctor may order diabetes testing. This is performed using a Fasting Plasma Glucose (FPG) test: a blood test performed to check the glucose levels in your blood after fasting overnight.

Our medical staff will discuss the preparations for this test with you.

DMV & Nursing Physicals.

Required exams

DMV and nursing physicals are available as part of our preventive care services. Bring any forms or paperwork required by the requesting agency or program to your appointment, and our staff will guide you through what to expect.

Nutrition Education.

Lifestyle support

Nutrition education is offered for patients with diabetes, high cholesterol, or high BMI. Your physician will discuss what's appropriate for your situation during your appointment.

Disability & FMLA Forms.

Documentation

We complete disability and FMLA forms for our patients. Forms must be submitted online (not turned in to the practice in person), and FMLA and disability forms cannot be completed on the same request. Normal turnaround for completed forms is 5 to 7 business days. Expedited requests are available in 3 business days for an additional fee.

Standard fee is $30 per form, $15 for an updated form, and $50 for expedited requests. For questions during the form process, contact us at 256-289-8364.

What insurance do you accept?+
We accept BC/BS, Medicare, Aetna, UHC, Cigna, and a limited number of Medicaid patients. If you don't see your plan listed, call us at (256) 909-4004 and we'll let you know.
What should I bring to my first appointment?+
Bring your insurance card and a photo ID. Bring a list of all your current medications, including dosages. The New Patient Forms can be downloaded ahead of time from the Patient Information page so you don't have to fill them out in the waiting room.
What if I'm under 18?+
Patients under the age of 18 must be accompanied by a parent or guardian at the consultation visit.
How do I request prescription refills?+
Patients can message the office to request refills through the Patient Portal during regular business hours.
How do disability and FMLA forms work?+
