01 / First Visit

What to bring,
what to expect.

Please prepare the following before your consultation, to make the time of your consultation valuable. The information below comes straight from the items required at intake.

  • 01Your insurance card and photo identification
  • 02A list of medications you are presently taking, including the dosage
  • 03A list of any allergic reactions you've had to medications, anesthesia, pets, and insects
  • 04Your family medical history
  • 05A list of all past surgeries, including the dates
  • 06Your doctor's diagnosis information, if applicable
  • 07You will be responsible for your co-pay at arrival

03 / Office Policies

The practical parts.

Direct from our practice. Read these before your visit so there are no surprises on the day of your appointment.

i.

Office Visits.

We see patients ages 18 and older. We strive to see all patients in a timely manner. However, medical issues arise from time to time. Delays may occur and appointments may need to be rescheduled. We understand your time is valuable and your patience and understanding is appreciated.

ii.

Payment & Co-pays.

All co-pays, deductibles, and other payments are expected on the day of service unless prior arrangements have been made with the billing manager. Co-pays are determined based on the rate as set by your insurance provider. For your convenience, we accept VISA, MasterCard, and Discover.

iii.

Prescription Refills.

Our doctors are not in the office every day. Therefore, please be mindful of your prescription refills, and have arrangements made for any of your prescription needs before they last 5 business days. Additionally, medication refills will only be addressed during regular office hours (Monday to Friday).

iv.

Forms & Paperwork.

All co-pays, deductibles, and other charges are expected to be paid at time of service. We charge a fee to complete FMLA, disability, paperwork, payment of absence forms, family leave forms not completed during regular business hours.

04 / FMLA & Disability

To complete a disability
or FMLA form.

Click the link to the right to begin. Please DO NOT bring forms to the practice to turn in. Make sure to have your disability or FMLA form with you BEFORE logging in (you should have received these forms from your disability carrier or employer).

Normal turnaround time is 5 to 7 business days. Forms not submitted through this link could result in a slower turnaround. The cost is $30 per form, $15 for an updated form, and $50 for an expedited request (guaranteed 3-day turnaround). FMLA and Disability forms cannot be completed on the same request and must be submitted separately.
